4th Grade Math Intervention Assessments



These Math Intervention Assessments are perfect to assess your students during math interventions and to use during Math RtI.

Have you ever been told that you need to be implementing RTI for math? Do you know where to start? This is the reason I created the Standards-based progress monitoring. Providing evidence of students learning is crucial. Progress monitoring will allow you to document student growth and make sure that the interventions that you administer are working. Everything you need to assess and track students is provided here.

Each of the Common Core Standards contains A LOT of little pieces. I broke these down into the pieces that my students needed to master in order to master the entire standard.

For example, within 4.NBT.2, one particular student may not have difficulties comparing whole numbers but they may need additional assistance in writing numbers in other forms. This student doesn’t need interventions or assessing with comparing numbers, just writing names for numbers. For this reason, 2 separate assessments for 4.NBT.2 were written.

There are a total of 46 concepts covered within this resource. Each concept includes 10 probes.

A diagnostic assessment is provided in order for the teacher to diagnose students’ strengths and weaknesses.

Progress monitoring forms and graphs are provided in both printable and editable forms.

These assessments were created to align with the 4th grade Common Core Standards, but can also be used for below level 5th graders.
